What is a lavage?

Cytostatic  lavage of the abdominal cavity is a new method of treating patients  during and after surgery, originally developed for malignant tumors of  the peritoneum, but currently applied with good results during other  challenging surgical procedures. While surgery usually occurs when  removing the tumor to release cells, which can adhere to surfaces early  and subsequently grow. Additionally, when the operation can be removed  only visible structures, while the microscopic escape attention. To  avoid their new growth, it is necessary residues tumor as soon as  possible. For now, the most effective method of disposal is a new method  of lavage of the abdominal cavity known as "Hyperthermic cytostatic  intraoperative intraperitoneal lavage of the abdominal cavity," called  the international abbreviation "HIPEC" (or HITOC- lung lavage).

The  method is particularly suitable for the treatment of malignant tumors in the abdominal cavity (gastrointestinal and gynecological organs). The  principle of the lavage consists of filling the abdominal cavity of the by patient cytostatic solution and keeping the temperature around 43°C.  This process incorporates two important principles that are required to  destroy small tumor cells. Thermotoxicity, cytotoxicity and their  interaction with a multiplying effect. Principle of thermotoxicity: During  heating of healthy tissue cytostatic agent to a temperature of 43°C, it  is on blood circulation, so as to divert excess heat and thereby cools  down the cytostatic solution. Tumors pseudomyxoma type and the like is unlike normal tissue when heated cytostatic solution to a temperature  of 43°C, preventing covers its surface with a protective layer of  protein (heat shock protein), as opposed to healthy tissue by the tumor  tissue could dissipate heat. The protein layer of tumor tissue however  makes the immune system, which it can be identified as an intruder  and effectively combat it.

Cytotoxicity is a chemical treatment of small microscopic tumors by strong solution cytostatics (800-1000x more concentrated than for intravenous process). The intention is elevated  concentration of a cytostatic that as lavage liquid of thermotoxicity  received in suprathreshold concentration  to the "root" of the tumor. The critical period for action of the  cytostatic solution is 30 to 35 min from the beginning of its  application, when the cytostatic solution saturated with (runs) and how contact with the tumor as well as healthy cells lapped (lavage)  internal tissues. After a rest period lavage, another 60 minutes, it  applied the principle remains thermotoxicity. Given the natural  thermoregulation of the human body, the cytostatic solution into the  abdominal cavity continuously cooled by actual body temperature of the  patient. For these reasons, the cytostatic solution is recirculated  through a special heat exchanger in which it is reheated to a  temperature of 43°C. The process of effective interaction heated  cytostatic solution is practice set for around 90min (For more  information on www.surgicaloncology.com or www.peritonectomy.com).
